The documents listed below are available for your convenience. We have provided the forms to facilitate a process by which an individual can make arrangements without visiting our offices. While we encourage people to come into our facilities we understand that there are times when the individuals making the funeral arrangements may live out of town or be physically unable to visit. You may print these forms and fill them out by hand.

These are basic forms that can be used for cremation or funeral services. In addition to the forms, we have provided copies of the State of California’s Consumer Guide to Funeral & Cemetery Purchases (English & Spanish versions) for your perusal. When you contact our office, our staff will let you know which forms you will need.

Description of Forms

• Authorization for Disposition with or without Embalming
  This form will tell our staff whether you would like the deceased to be embalmed. Embalming is not required by law. If you choose a service with an open casket or a service in a chapel embalming will be beneficial.

• Authority to Cremate and Order for Disposition
  This form provides information on the cremation process. The form must be filled out by the person(s) authorized to determine the final disposition of the deceased. Additionally, the form allows the authorized individuals to provide information to the funeral home to assist in completing necessary paperwork.

• Declaration for Disposition of Cremated Remains
  This form is required by the State of California if the deceased will be cremated.

• Witness Cremation Request Form
  We offer clients and their families/friends the opportunity to witness the cremation process in our facility. This form must be completed by the person legally authorized to control the disposition of the deceased.

• Order of Release for Fresno County Form
  This form authorizes the Fresno County coroner’s office to release the decedent to the funeral home.

• Authorization for Release of Remains of the Decedent Form
  This form authorizes hospitals, nursing homes and other funeral homes to release the decedent to the named funeral home.

• Preneed Disclosure Form
  Through this form the funeral home discloses whether there is a written preneed arrangement on the decedent. By law, funeral homes are required to disclose the existence of preneed funeral agreements.
